સમસ્યા

• કન્સેન્ટ ફોર્મ પ્રિન્ટ કરીને, કાગળ પર સાઇન કરીને ફરી સ્કેન કરવામાં આવે છે, જેનાથી સાઇન થયેલું વર્ઝન ફાઈલમાં રહેલા વર્ઝન સાથે મેચ ના થવાનું ગેપ રહી જાય છે.
• અધૂરા ભરેલા કે સાઇન વગરના ફોર્મ રેકોર્ડમાં ઘૂસી જાય છે, જેનાથી દર્દીએ બરાબર શેના માટે સંમતિ આપી એ અંગે વિવાદ થાય છે.
